Live license refresh on status check

• get_license_status now re-validates against the server, so a mid-session dashboard upgrade is reflected and the frozen usage block is lifted (or re-applied) without an MCP restart.
• `refreshLicense()` bypasses the per-process memo but keeps the last-known verdict as an offline fallback (won't drop to free if the API is unreachable).
• Usage-cap block message now shows live plan pricing instead of a hardcoded $9/mo.
