Task Passport continuity and ergonomics patch. Adds Task Passport update flow, exposes task_update through MCP, preserves repo-root write scope, protects active current passports from accidental replacement, validates task risk consistently, and tightens generated agent instructions toward compact ledger cadence. Verified with npm test, MCP smoke, npm pack dry-run, privacy scan, and diff check.
