Retrospective session analysis (bench/analyze-codesage-quality.py on the
last 7 days) found that 59% of find_coupling MCP calls returned an empty
array. Empty responses have three distinct causes, and the bare
Vec<CoChangeEntry> shape gave the agent no way to tell them apart:

1. File has no row in git_files — not tracked, no commits yet, or the
   caller passed a path the index doesn't recognize.
2. File has commits but no co-change pair above the min-count=3
   threshold — "this file historically changes in isolation."
3. Same as #2 but file has fewer than 3 commits total, so the threshold
   mechanics explain the emptiness more directly.

Replaces the return type with:

      pub note: Option<String>,  // only populated when coupled is empty
  }

`note` is populated only when coupled is empty and quotes the specific
cause. `file_indexed` and `file_commits` are always present so a thin
result (fewer than `limit` entries) is still interpretable. MCP tool
description updated to tell the agent to index into `.coupled`, not
treat the response as an array. CLI `codesage coupling` prints the
explanatory hint when there are no results.

Breaking change for callers that treated the response as a bare array:
the CLI's `--json` output, MCP clients, and any external script. Since
the tool is not widely embedded outside this repo and v0.5 hasn't
shipped yet, the clean break is preferred over a dual-shape bridge.

4 new tests cover the three empty-reason cases plus the populated-with-
state case.
