Breaking: default input normalization and guarded decode — v5.0.0.

All six runtime items, TDD'd: the echo bug clients ship today is fixed (guards ported from the validated Python harness), inputs normalize by default, downloads stream progress, tiers auto-select with warm-up, the index caches for offline-first with stale eviction, and decode exposes per-step confidence. Breaking changes are the default normalization + guarded decode -> 5.0.0. 247 tests green.
