release(v2.13.0): bump version, changelog, fastlane metadata

Bump versionCode 46 → 47, versionName 2.12.1 → 2.13.0.

Add CHANGELOG.md and CHANGELOG.de.md entries for v2.13.0 covering the
regrouped settings screens, title-only note cards, the hideable card
timestamp and note icon, the optional scroll-to-top on un-check, and
the checklist parser, animation and widget fixes.

Add fastlane changelogs 47.txt (EN/DE, ≤ 500 chars each).
Update README footer version badge to v2.13.0.

No translation updates in this release: the only open Weblate PR (#111)
adds an empty values-fa/strings.xml and stays unmerged.
