Add fast, integration, browser, and platform test lanes

Summary

• split quick, integration, coverage, browser, and host-shell checks into named lanes
• enforce per-file coverage floors for the core parser, target, note, and presenter paths
• run pull request lanes on Linux and check macOS and Windows shell rules on a set schedule
• keep Chromium setup separate from the browser test command
