Retire the divergent completed-plan stack

Closes #23.

Summary

• replace false completed-plan claims with reconciled or rejected decisions and current PR evidence
• record the audit of seven obsolete plan worktrees, preserved branches, and 5,651,140 KiB reclaimed
• add a ledger test that rejects stale replacement commits and unproved merge claims
• leave doctor code to PR Report doctor capabilities separately #44 instead of copying it into this PR
