AllCode v0.3.0

Add installed-agent discovery to /add agent, reviewed local adapters for additional CLIs, shared skill installation, and startup responsiveness improvements. Custom adapters live outside the package so updates preserve them. Install the attached tarball with npm install -g ./allcode-0.3.0.tgz.
