Registry-related changes:

  • IVORN -> IVOID (and explaining the rationale)
  • recommending the question mark as local id separator now

I also took the liberty of adding myself to the author list, since
I've now touched a critical numbrer of lines.

This is intended to be applied on top of PR #23.

\tt and \bf will stop being supported in various LaTeX dialects, and
they're ancient anyway.

What's a bit doubtful here: I'm marking up the <Whatever> thing via
\verb (where I can; else it's \texttt) and actual xml names and
attributes via ivoatex xmlel.  I think that's fine because the <> don't
really belong into an xmlel.

Also, shortening a few of the absurdly long author lists.

This is supposed to be applied on top of PR ivoa-std#22.
