A table-of-contents entry no longer becomes a section heading. A line with a dot leader and a page number
("2.5.1 Harmony Chat Format . . . . 6", "HOST Interface ........ 11") is a contents entry, whether it stands on its own
line or the reader marked it as a heading. Before, each such entry became a second, empty section in front of the real
one. The newer PDF extractor puts every contents entry on its own line, so a PDF with a contents page got one false
section per entry.
