Repeated ToolCallEvents for the same call.id now update one card in
place instead of creating duplicate widget keys. Consecutive
ThinkingEvents and successive Plan/Citation snapshots collapse to a
single live widget.

Fixes assertion: 'Duplicate keys found ... timeline-tool-<id>'.
