Visual overhaul

• Squared borders everywhere — 1px flat borders on buttons, cards, frame, minimap button
• Custom scroll bars — thin track with draggable thumb, no arrow buttons
• Header bar — visual strip across the top with all action buttons
• Bigger minimap button (32px, squared)

New features

• Preview mode (/ks preview) — generates a fake 25-man raid with randomized M+ data for testing the UI without needing a group
• Apply button — moves players into native raid subgroups via SetRaidSubgroup() and announces assignments in raid chat. Works for all raid members without them needing the addon.
• Auto-scan — roster refreshes automatically when members join/leave (GROUP_ROSTER_UPDATE) and on first window open

Bug fixes

• ? button now clickable — all title bar buttons are children of the header bar, no more click-blocking
• Roster shows data on first open — auto-scans instead of showing empty until filter clicked
