1.4.13 — Fix add/discovery loop

Fixes an add → 'Success' → vanish → rediscover loop that could happen when a device misses its very first poll during setup (e.g. a slow/busy device). The integration no longer ghost-removes a freshly-added entry; orphan cleanup now only runs at HA startup. Entities come up unavailable until the device answers.
