Code Quality Improvements

Address all ObsidianReviewBot code review findings:

• Remove unnecessary type assertion in applier.ts
• Fix async methods returning void instead of Promise
• Use void operator for intentionally ignored Promises
• Use sentence case for UI text (Auto improve, Generate template)
• Fix error throw to always throw Error object
• Fix exhaustive type check in provider switch
• Use setHeading() instead of createEl for headings
• Replace inline styles with CSS classes
• Remove unused imports and variables
