v5.13.10 — change_guard 健康基线锚定在无持久化 git 身份仓库可成功（登记册 #3）

承接登记册「多 Skill 成效与调度审查」#3：coderef_change_guard action=anchor 对 Coderef-Ai-master 失败 Committer identity unknown（仓库无持久化 user.name/email，git tag -a 需 committer 身份）。

修复：新增 _HEALTH_IDENTITY（-c user.name=CodeRef-AI -c user.email=coderef@local），anchor 的 commit/tag 统一带临时身份执行。不写 git config（不违反 NEVER update git config 铁律），任意已存在仓库均可锚定，无需配置 git 身份。

验证：临时仓库无持久化身份场景 anchor 成功打 coderef-health-* tag；对 Coderef-Ai-master 补锚成功，health_baseline 不再为 null。全量 164 用例通过。
