v0.68.2

Historical review migration now processes 100 reviews per batch, reducing startup memory use and fixing the migration memory problem in 0.68.1. Completed review updates survive interruptions, and startup logs show progress. Review content, IDs, and open/closed state are preserved.

Upgrade directly from 0.67.0 or 0.68.0 to 0.68.2. Migration runs automatically. If the 0.68.1 review migration already completed, there is no remaining review conversion work.
