Session-scoped account routing

Each coding session now remembers its own active account. Switching accounts mid-conversation no longer bleeds into other sessions — the selection is persisted in the session branch and automatically restored on reconnect.

Account architecture overhaul

The monolithic accounts.ts has been decomposed into focused modules:

• accountVault — file-backed credential storage with atomic read/write and file-locking
• accountRouting — token resolution, refresh deduplication, and route construction
• accountMigration — seamless one-time migration from the legacy config format
• sessionAccountSelection — per-session account state management

Removed legacy shims

• Cursor tool shims (src/tools/*) — file, glob, search, shell, read, web-search, and rendering shims have been fully removed (~1,200 lines). The host now provides these natively.
• Vision routing (src/vision/*) — removed in favor of native Composer image input, eliminating ~530 lines of describe/cache/register code.

Fixes

• Dashboard account-action buttons now work correctly after the vault refactor
• Hover ring corners on dashboard account cards match the card border radius
• Token refresh timing corrected to avoid premature expiry errors
