Patch Changes

fix: surface config load failures instead of silently falling back to defaults

Config files that exist but fail to import (e.g. .ts config in a CommonJS project without "type": "module") now produce a warning to stderr and in _configWarnings on the config object. Previously the error was silently swallowed, causing forge-ts to run with default config while the user's config file was ignored with no feedback.
