Refreshes logical commit plans after partial commits

This commit exports the logical commit change unit type, adds stable hunk keys, and reworks diff helpers so commit plans can be rebuilt from surviving changes. It adds planner logic to refresh an existing logical commit plan from a new diff and updates the extension flow to reuse that plan after a proposal is committed. It also removes the stale-plan state from the logical commits webview so proposal actions stay available while the refreshed plan is shown.

Raises the default logical commit limit

This commit updates the package configuration to increase the default value from 100 to 200. The extension will now start with the higher default limit without requiring any user configuration changes.
