derive command names from the operationId

feat(model)!: derive command names from the operationId

Breaking: every generated command name changes. Regenerate your clients and update call sites.

Names are the spec's operationId, kebab-cased. Operations without a usable one get a <method>-<path> slug. Word order and phrasing are now the spec author's choice, not the generator's — a spec that names things verb-first gets verb-first commands.

Migrating: run http-gen preview ./spec.yaml to see the new names before regenerating. If you to customize the naming logic, --name-fn gets {raw, params, derived} and can rebuild any naming scheme you like.
