Fixed silent opencode update failures, which had two causes: old npm shims shadowing the official binary in PATH so version checks returned stale versions, and running opencode processes preventing the installer from replacing the executable on Windows. update-all.ps1 now stops any running opencode.exe before running the official installer, because Windows cannot replace running executables. update-all.sh now cleans up npm shims before its version checks, probes the full binary path $HOME/.opencode/bin/opencode instead of relying on PATH, and removes old npm shims from $NPM_CONFIG_PREFIX/bin and $APPDATA/npm on Windows Git Bash, so npm shims can no longer cause false version detection.

The opencode section of update-all.sh was refactored to match the bootstrap approach: clean npm and bun shims first, then update only if the binary exists at the official installer location $HOME/.opencode/bin/opencode. deploy.ps1 gained verbose script deployment logging, on by default, that shows the source timestamp and the destination timestamp before and after each copied script, which makes deployment issues diagnosable.
