Security fix
MemoryMdAdapter now catches memory-file edits made while no watcher was running.
baseline() used to trust whatever was on disk, so an edit to MEMORY.md, CLAUDE.md or AGENTS.md made while the agent was down went unreported after a restart. baseline() now treats the ledger's last recorded write for each file as the trusted state, so the next scan() reports an offline edit. A file the ledger has never seen is recorded the first time it's seen, so a cold start stays distinguishable from a tampered file in the audit. (#31)
Measured with the agmi at-rest suite (eight storage-level edits, T1–T8): across a restart, 0/8 were reported before the fix and 8/8 after. The process-alive row stays 8/8.
Behavior change: baseline() now returns the list of records it wrote, not None.
Release pipeline
Release assets are uploaded once each, which fixes the failed upload after the action-gh-release v3 bump. (#32)
