Proactive login

• Login walls are handled by the agent: vault credential through bitwarden-use (chrome-use auth login --bwu, passkeys preferred), then the second factor from wherever the site sent it.
• New code command: newest SMS code (message-use) or email code (mail-use). --wait only accepts a code that arrives after it starts, --from narrows to one sender, --via sms|mail picks one source. Codes are printed, never stored.
• A locked vault is unlocked automatically: bitwarden-use unlock --keychain first (no prompt), else the pinentry dialog. New vault-unlock command. vault-setup now suggests unlock --keychain-store after the first login.
• vault-status reports code_sources (message-use / mail-use installed).
• Signing in to the user's own existing account on a verified domain no longer needs per-submit approval; registration, payment and security changes still do.

Fixes

• ValueFormatTests sat after the __main__ block and never ran under python3 tests/test_profile_use.py.
• Stale rbw / brew / cargo wording in CLI help; duplicated vault config parsing merged.
• Codex display name was still "Personal Autofill".
