🔧 Changed

• Discussion close now shows a dedicated gate when a review is already in flight, telling you plainly that answering "yes" just waits for it rather than starting a new one.
• The mandatory pre-conclusion review gate drops its dynamic reason text in favour of fixed per-case wording, so what "yes" does is always unambiguous.
• Research sign-offs that leave a topic open (e.g. "picking this up later") are now treated as a pause rather than a completion signal, and resuming skips redundant context-gathering.
• A follow-up question that reshapes an existing research thread now updates that thread instead of adding a duplicate row.

🐛 Fixed

• Discovery session logs are now read with the proper file-reading tool instead of being treated as plain text, avoiding truncation on large logs.
