⚠ BREAKING CHANGES

  • skills: codefall-design is now codefall-plan, and the documents it writes moved from docs/designs/DESIGN-NNN-slug.md to docs/plans/PLAN-NNN-slug.md, with the Task Plan section now Tasks, the Design ref now Plan ref, and the design-revision and design-amended labels now plan-revision and plan-amended. A project with documents under docs/designs/ keeps them; run /codefall-upgrade and take the legacy-documents items so the renamed verbs read them.
