• Make Connection.ensure_closed() tear the connection down locally instead of raising
  OperationalError(2006) when the server already closed the stream, so pool invalidation of a
  dead connection (e.g. SQLAlchemy do_terminate) no longer fails.
• Fix SSDictCursor returning tuples instead of dictionaries, and honor custom dict_type
  subclasses when fetching unbuffered rows (#164).
