QuotaBar v0.4.0

QuotaBar v0.4.0 adds Grok Build monitoring, Codex weekly API-equivalent value estimates, and independent provider trays. It also fixes Grok value accounting to use ccstats per-inference pricing with exact billing-window timestamps.
