Agent Nudge v0.4 adds reversible Live Connect for Claude Code, Codex and OpenCode.

Honest limits:
- enforcement covers enabled and trusted provider hooks/plugins only
- hooks fail open when the local daemon is unavailable
- unsigned Windows binaries may trigger SmartScreen
- hard-process connector recovery is not automated yet
