Notifications on pomodoro start, focus completion and break completion — previously only focus completion notified anyone.

Chatlist summary updates every minute instead of every five, and refreshes the moment the app is reopened, so the remaining time shown is current rather than a stale snapshot. Backgrounding the app marks the countdown unwatched (💤) so a frozen number isn't read as live.

Fixes the tomato counter counting each completed pomodoro twice, which also left two devices showing different totals.

The footer now explains that keeping the app open is what allows the chat to be notified, and appears only while a timer is running.
