Database Migration Safety System

Defense-in-depth system to prevent forgotten database migrations from causing test failures.

Phase 1: /plan (Step 0.5) - Early detection via keyword pattern matching
- Scans spec.md for schema-change indicators (store, persist, table, column, etc.)
- Generates migration-plan.md artifact with change classification, schemas, and SQL
- Sets has_migrations: true flag in state.yaml
Phase 2: /tasks (Step 1.5) - Migration task generation
- T001-T009 reserved for migration tasks (P0 BLOCKING priority)
- Assigned to database-architect agent
- Layer-based execution ensures migrations complete first
Phase 3: /implement (Step 0.6) - Runtime enforcement
- Pre-flight check blocks execution if migrations pending
- Configurable strictness: blocking (default) | warning | auto_apply
- Supports Alembic, Prisma, and generic migration frameworks
New scripts and templates:
- check-migration-status.sh - Detects pending migrations across frameworks
- migration-plan-template.md - Artifact template for migration planning
- migration-detection.md - Reference documentation for detection patterns
User preferences: New migrations configuration section
- strictness - How to handle pending migrations (blocking/warning/auto_apply)
- detection_threshold - Keyword score sensitivity (default: 3)
- auto_generate_plan - Generate migration-plan.md during /plan (default: true)
- llm_analysis_for_low_confidence - Use LLM for ambiguous detections (default: true)
