Critical fix: Phase 3 was stuck processing only the first task per pulse because _dispatch_ai_worker referenced the removed worker_pid column in its UPDATE statement. Same root cause as v2.127.1 (PR #2275). Phase 3 should now process all 20 eligible tasks including 8 PRs waiting for review/merge.
