Fix: Always regenerate pulse plist on setup.sh upgrades

Root cause fix for stale plist problem. Previously, setup.sh detected an existing plist containing pulse-wrapper and skipped the entire install block — meaning new env vars, removed vars, and threshold changes were never applied to existing installs.

Changes

  • setup.sh now always regenerates the pulse plist on every run
  • User prompt only appears on first install; upgrades regenerate silently
  • Ensures all pulse-wrapper improvements (no timeout, 30-min stale threshold, dynamic concurrency) are actually deployed

Context

This completes the pulse reliability series:

  • v2.134.2 — pulse-wrapper, orphan cleanup, dynamic concurrency
  • v2.135.2 — remove hard timeout, increase stale threshold
  • v2.135.3 — ensure setup.sh actually deploys the above to existing installs
