fix: prevent pulse agent from closing supervisor health issues

Two-layer fix for the churn cycle where the pulse LLM closes supervisor health issues, then pulse-wrapper.sh recreates them on the next cycle:

  1. pulse.md Hard Rule 11: explicitly forbids the pulse agent from closing or modifying issues with the supervisor label
  2. pulse-wrapper.sh prefetch filter: removes supervisor, persistent, and quality-review labeled issues from the pre-fetched state injected into the pulse prompt
