Expanded Conversational Memory Lookup (t1387 follow-up)

Extends the memory lookup guidance from v2.149.0 with additional remote knowledge sources:

  • Tier 3 → Tier 3 + Tier 4 split: Targeted remote (need a number) vs broad remote (search/explore)
  • PR review comments: gh pr view --comments and gh api pulls/{num}/comments for inline review feedback
  • GitHub search expanded: gh search prs and gh search code alongside gh search issues
  • GitHub discussions: Long-form design discussions, RFCs, community Q&A
  • GitHub wiki: Architectural decisions, onboarding guides, historical context

The full progressive discovery chain is now: local cached (memory, TODO) → local indexed (git, transcripts, session DB, observability) → remote targeted (issue/PR comments) → remote broad (search, discussions, wiki).
