Extend comment-based approval to external contributor PRs (t1421): External contributor PRs now get needs-maintainer-review label alongside external-contributor, with approval instructions in the flagging comment. Maintainers can comment approved or declined: <reason> on both issues and PRs. The pulse handles each type appropriately — issues get auto-dispatch, PRs get their review gate removed for merge.
