fix: pulse monitoring loop heartbeat prevents watchdog kill

The wrapper's progress detector killed productive pulse sessions during the monitoring loop's sleep 60 because no log output was produced. The LLM now echoes a heartbeat line before each sleep, keeping the watchdog happy while retaining full LLM control of dispatch decisions. This enables continuous slot refilling instead of batch-and-idle dispatch.
