fix: reap stale TTY-attached opencode processes in pulse cleanup

OpenCode enters idle file-watcher state after task completion, never exits. New cleanup_stale_opencode reaps processes older than 4h with <5% CPU, with SIGTERM→SIGKILL fallback. Reclaims memory, TTY slots, and terminal tabs.
