• Simplification cycle infinite loop (t1754): Three compounding bugs caused the simplification scanner to endlessly re-flag files. Hash algorithm mismatches (6 entries), fragile GitHub API backfill, and lack of convergence criterion meant files like hashline-edit-format.md were simplified 6+ times. Fixed by: correcting all state hashes to SHA-1, replacing API-based backfill with local git hash-object refresh, and adding a pass counter that marks files as converged after 3 passes. Pass counter resets when files are genuinely modified by non-simplification work.
