Fixed

  • Observability plugin: replace per-query execSync with persistent sqlite3 process — sqliteExec() forked a new /bin/sh + sqlite3 process for every INSERT/UPSERT, causing hundreds of concurrent forks under sustained load and cascading ETIMEDOUT failures. Replaced hot-path calls with a single persistent sqlite3 child process using sentinel-based stdin/stdout protocol, write queue serialization, and auto-respawn. Init-time operations retain synchronous execution. (#15978, closes #15957)
