Fixes

  • Simplification scanner state format mismatch — 33 entries were stranded at top-level instead of under .files, causing duplicate issues for already-simplified files every pulse cycle
  • Permission gate on simplification scans — non-collaborator instances can no longer file simplification issues on repos they don't have write access to (GH#16786)
  • Conditional needs-maintainer-review label — skipped when the authenticated user is the repo maintainer
  • Cleaned state schema — removed unused status, note, lines_before/after, action, issue fields. Canonical: {hash, at, passes, pr}
  • Scanner resilience — hash lookup now falls back to legacy top-level entries

Closed 17 spurious issues created by non-collaborator instances.
