Deterministic NMR gate (t1894 follow-up)

Completes the cryptographic approval gate by removing needs-maintainer-review issue data from the LLM pulse's state file. Previously, an LLM could see NMR issues and potentially attempt to bypass the gate.

  • NMR triage data now written to a separate file only the deterministic shell code reads
  • LLM docs explicitly state NMR issues are handled deterministically
  • Removed legacy 'Comment-based approval' instructions from pulse sweep docs
