fix: make dispatch claim comments persistent and respected as primary dedup lock (GH#17503)

• Claim comments are never deleted — they form the audit trail of every dispatch attempt
• 30-min TTL (was 120s) covers a full worker lifecycle
• Removed active-claim-state gate, pgrep escape hatch, and deferred claim deletion
• Completion-comment awareness cancels lock early after worker done signals
• Prevents duplicate dispatch that produced 2 PRs for the same issue
