feat: try session continuation before killing stalled workers with activity (GH#17648)

When the watchdog kills a worker that was making progress (stream drop mid-session), the system now tries to resume the session up to 2 times before falling through to provider rotation. This preserves worktrees, file changes, and partial implementation instead of starting from scratch.

  • Exit 124 + activity → resume session (up to 2 attempts)
  • Exit 124 + no activity → rate_limit rotation (unchanged)
