fix: default simplification issues to tier:standard instead of tier:simple

Simplification tasks require reading files, understanding structure, and preserving institutional knowledge — standard-tier judgment, not Haiku-tier prescriptive execution. Previously, all simplification issues were labeled tier:simple, wasting the first dispatch attempt on a model that couldn't handle the task.
