feat: auto-create worktree, provide file paths, log empty-result gaps

Three improvements to worker resilience based on analysis of gpt-5.4 failure sessions:

  1. Auto-create worktree: pre-edit-check.sh now creates the worktree deterministically on exit 2 instead of signalling and hoping the model knows how. Returns WORKTREE_PATH + exit 0.
  2. Key file paths in contract: headless workers get resolved paths for the 5 most-needed files, plus guidance to retry on empty results instead of stopping.
  3. Empty-result gap logging: premature_exit now scans for empty tool results and logs to worker-empty-results.log. Each is a closeable gap for future path hints.
