fix: replace codex fallbacks with general-purpose gpt-5.4 models

Codex models (gpt-5.3-codex) are code-completion, not agentic — zero tool calls when dispatched as headless workers. Swapped to general-purpose models:

  • sonnet fallback: gpt-5.3-codex → gpt-5.4
  • opus/coding fallback: gpt-5.4 → gpt-5.4-pro
