fix: add lifecycle tracing to diagnose continuation retry failure (#18074)

Adds trace points to _invoke_opencode and _execute_run_attempt to diagnose why the continuation retry mechanism (GH#17436) has never fired in 100+ worker sessions. Post-execution messages (db_merged, WORKER_EXIT_DIAGNOSTICS, premature_exit) are absent from every worker log, meaning the process tree dies before reaching the retry loop.
