Workflow-guard readback timeouts no longer disable the guard for the rest of the session after an operation has succeeded. Status refreshes now report timed-out probes in unconfirmedReadbacks, while completion attempts return a retryable readback-timeout and leave the unit active. This lets a slow git or gh readback recover without treating an unverified result as confirmed. Guidance allows at most two completion retries before reporting the timeout; other readback failures still fail closed. (#1088)
