Fixed a bug where apply_patch operations targeting an absolute path inside a registered nested worktree would fail closed to guard-unavailable when no workdir argument was given. The root cause: deriveApplyPatchTarget seeded the shared-root set with the parent checkout root, so even though per-file targets correctly resolved to the worktree, a mismatch between the parent and worktree paths made sharedTargetRoot return undefined.
The fix derives the shared root from the patch-file targets alone when workdir is absent and the patch names files, matching deriveFileOperationTarget's existing handling of write/edit operations. Behavior is otherwise unchanged: an explicit workdir, or a patch with no file targets, still seeds the workdir root, preserving both the multi-worktree spanning guard and the parent fallback for fileless patches.
