Dim and reverse text now show in the preview. carve draws table borders and quote bars dim and highlights in reverse; before, tables looked heavy and highlights showed as plain colored text.

Also: a contributing guide on adding a renderer, its demo tape and the renderer quirks worth knowing, and make demo now only records the gifs that are missing.

Carve support! Documents in Carve (.crv) now render beside the source with carve --ansi. .crv files get a carve filetype, since Neovim does not have one yet.
